Not very often I think, but this could happen at any time.

I don't think it's really related to other windows popping up or not. Simply
a fullscreen DirectX application may "lose" the primary surface from time to
time on NT-based Windowses, while it doesn't seem to be the case on Win9x.
That's not clearly stated in the latest DirectX SDKs, but I remember reading
such a warning in the DirectX 3 SDK.
