[AD] Patch to add constructor stuff to load_bitmap, save_bitmap

Hi,

Here is a patch which wraps src/readbmp.c (load_bitmap(), save_bitmap()
et al.) in a constructor:

This should slightly decrease the size of certain statically-linked
binaries. Also, I changed the `register_bitmap_file_type_init()'
function to have a prepended underscore, to fit in with other globally-
visible undesirables :-)
