[AD] ex_pixelformat broken?

Is ex_pixelformat currently broken?  With some certain src/dest pixel
formats either I see some junk (on one machine), or the program crashes
(with another).

PS. Can we spend a few more seconds on log messages than this:
"Implemented new locking mechanism as discussed on AD."  It took me
ages to locate the correct thread today, let alone in a few years time.
