[AD] clean up after Mac port

Done for files outside `mac' directory, which involved reindenting
some lines, removing some casts which are no longer pertinent, etc.
I may have broken something though.

There are still some pointless casts in `fli.c', among other things
in other places, but it's mostly fine now. :-)
