[AD] TODO list

I read some of the TODO list  and found each entry complete enough to 
understand the problem; yet no extra info about who/when/why/what/how 
the problem occurred, which leaves me unable to pursue them.

Is there a more documented TODO list ?

Some of the entrys that mention "investigate error ..." , not enough 
info for me to chase up.

Have we had the "lets use a bugzilla style error form" discussion ?
