Due to the way the makedoc is a simple scanner, the index is
generated at the end when all symbols from the documentation have
been read. There is certainly a second post processing pass for the
documentation, so if you feel like it you could hack the code to
move it there and provide a new documentation embedded parameter
like @html_index_at_top to generate the index at the top.
Books tend to have such indices at their end. Besides, in HTML its a
matter of having a bookmark of a different HTML file, if you access
it frequently.
