[AD] 4.0.1 release

I've uploaded what should be the definitive packages for the 4.0.1 release:

It would be nice if they were tested (compilation/installation) on the
maximum number of platforms.

Here are the 8 main platforms:
- DJGPP: ok (2.03, gcc 2.95.3)
- Mingw32: ok (1.1, gcc 2.95.3)
- Borland C++: ok (5.5)
- QNX: ok (6.1, gcc 2.95.2)
- Linux: ok (Mandrake 8.0, gcc 2.96)
- Watcom: untested
- MSVC: untested
- BeOS: untested
