[AD] slightly improved create_light_table()

Hello,
This patch makes create_light_table() produce the exact palette for the
255's entry in the table. It did not always to this before because of
the inaccuracy of rgb_maps, and because some palette entry may be
duplicated. It also rounds the pixel values more correctly if the
rgb_map is used, and it's a bit faster (takes 20% less time on my
system).
