I've attached a patch that will hopefully solve all the problems related to
the mouse cursor under Windows. It's a true fix, as it eliminates all the
successive hacks I coded in order to hide the various effects of the
underlying problem.

I extensively tested it, including with AllegroGL 0.0.20 and with a custom
Windows pointer.
