[AD] GFX_SAFE patch

Hello.

This patch modifies GFX_SAFE behaviour according to todo.txt, updates all
examples, demo and tools. setup has been modified back to GFX_AUTODETECT:
using GFX_SAFE with it is just as complicated as with the demo due to the
palette tricks played there.

According to this, the corresponding entry in todo.txt can be erased or
transformed into GFX_SAFE testing, or behaviour spread to graphic windowed
drivers (ie: they could use the desktop color depth if GFX_SAFE is used,
to avoid color conversions).

PD: Long filenames are nice :)
