Initial Comment:
We need to expose _AL_MALLOC as al_malloc(), etc. Easy.
A bit harder is sorting out strdup-like functions and documenting where the user should be calling al_free() instead of free().
Update the addons.

Also make al_set_memory_management_functions use a vtable structure instead
of so many arguments.
