Add display field to mouse state struct to tell which window the mouse cursor is over.
Consider deleting al_set_mouse_xy and any other function which don't work well in multi-window environments.

Implemented in svn, with one omission (noted in the commit log): the mouse
leaving the display is recorded immediately, while we may want to defer
that if a button is held down until the button is released.
