[AD] mode selector rewrite!

i promised you some code, so here it is... i have tested it with mingw and
it seems to work fine.

there are two things that doesn't work yet. when updating the selectors, the
list size in the dialog struct seems to not get updated. and if you change
the gfx driver the selected mode entry and the selected color-depth entry
should be set to zero. likewise if the mode is changed the selected
color-depth entry should be set to zero. i haven't used the allegro GUI
system much so it would be nice to get some help with this.

can i just put D_DIRTY in the dialog struct to get rid of the list-size not
updated problem?
