I got it to compile by putting this line in at line 148 in ddraw.h:

         DECLARE_HANDLE(HMONITOR); // mcf

I also use djgpp make to compile the bits that mingw32 make won't make.  I 
use ming make until the build crashes, then use djgpp make until it 
crashes, then use ming make again until the whole thing is built.
