In DOS, the driver list has DIGMID above everything else except
AWE32, whereas in Windows the driver list is generated at runtime,
and never mentions DIGMID at all.  This is probably good, since
newers cards already have wavetable functionality under Windows. It
might be possible to check if a card has wavetable support, and use
DIGMID in preference if not, but that seems a little messy to me.
